Statement of Health or Evidence of Insurability for Group Life and Disability Plans

What Is a Statement of Health (SOH)?

A Statement of Health (SOH)—also known as Evidence of Insurability (EOI)—is a health questionnaire required by insurance carriers when an employee elects coverage above the guaranteed issue (GI) amount for voluntary life, short-term disability (STD), or long-term disability (LTD) insurance.

The SOH form helps carriers assess an individual’s health status to determine eligibility for the requested coverage. It typically includes questions about medical history, lifestyle habits (e.g., smoking), and recent treatments or diagnoses.

Completing the SOH is essential to activate coverage beyond the GI amount. Employees should submit the form directly to the insurance carrier, not to their employer or broker. Online submission is often the fastest and most efficient method. If the employee does not receive the electronic questionnaire via email, please have them complete the insurance carrier’s form.
